However, certain "red flag" symptoms indicate a potentially serious underlying condition requiring urgent evaluation by a neurosurgeon.
Red Flag #1: Bladder or Bowel Dysfunction
If back pain is associated with difficulty urinating, loss of bladder control, or loss of bowel control, this is a MEDICAL EMERGENCY called cauda equina syndrome. Call Dr. Waqas Mehdi or go to the ER immediately. Delay can cause permanent paralysis.
Red Flag #2: Weakness in Both Legs
Weakness in one or both legs suggests nerve root or spinal cord compression. Progressive weakness requires urgent MRI and likely surgery.

New or worsening back pain after trauma may indicate a spinal fracture. Do not assume it's just a muscle pull — get X-rays and a medical evaluation.
Red Flag #4: Night Pain That Wakes You Up
Back pain that wakes you from sleep (not just when you change position) can indicate infection, malignancy, or inflammatory disease — not mechanical back pain.
Red Flag #5: Back Pain with Fever
The combination of back pain and fever suggests spinal infection (discitis, osteomyelitis, epidural abscess) — a serious condition requiring immediate evaluation and antibiotics.
Red Flag #6: Back Pain in a Cancer Patient
Back pain in someone with a history of cancer (breast, lung, prostate, kidney) must be evaluated for spinal metastasis.
Red Flag #7: Age Under 20 or Over 50 with New Back Pain
Back pain starting in childhood or adolescence is less likely to be degenerative and more likely to have a specific cause. New onset in the elderly warrants evaluation for fracture, cancer, or infection.
Red Flag #8: Unexplained Weight Loss with Back Pain
Significant unexplained weight loss accompanying back pain is a classic warning sign for malignancy or serious systemic illness.
